iCloud passwords and keychain does support OTP and auto fill. All of this can be accessed from the system settings and not through safari (though Apple really should make passwords it’s own app and integrate card and id info with it). It’s not a safari password manager. It’s just a current limitation in Mac OS that only safari can auto fill from it. On iOS it is universal.
